ABSTRACT

ObjectiveTo evaluate the clinical efficacy and safety of Yangxin Dawayimicol honey ointment (YDHO) in the treatment of insomnia with the syndrome of Qi stagnation and blood stasis. MethodEighty insomnia patients who met the inclusion criteria in the Department of Encephalopathy of the Third Affiliated Hospital of Henan University of Chinese Medicine from November 2019 to October 2020 were randomly divided into an experimental group (48 cases) and a control group (32 cases). The experimental group was treated with YDHO + Xuefu Zhuyu capsule simulators,and the control group was treated with Xuefu Zhuyu capsules + YDHO simulators for eight weeks. The changes in Pittsburgh sleep quality index(PSQI)score,traditional Chinese medicine (TCM) syndrome score,insomnia severity index (ISI),neurotransmitter indexes [γ-aminobutyric acid(GABA),glutamic acid(Glu),and 5-hydroxy tryptamine(5-HT)],serum inflammatory indexes [interleukin-6(IL-6)and interleukin-10(IL-10)], and safety index of the two groups were compared. ResultThe total effective rate was 97.83%(45/46) in the experimental group, higher than 68.75%(22/32) in the control group(Z=-4.292,P<0.01). The experimental group was superior to the control group in PSQI score,ISI score,TCM syndrome score, and sleep duration(P<0.05). The curative effects were equivalent between the two groups in shortening the time to fall asleep. The experimental group showed increased serum content of GABA,5-HT, and IL-10 and reduced content of Glu and IL-6,with few adverse reactions (P<0.05). ConclusionYDHO is effective,safe, and reliable in the treatment of insomnia with Qi stagnation and blood stasis syndrome.

ABSTRACT

Objective:To study the distribution of ischemic stroke treatment with data mining technology and evaluate its clinical efficacy. Method:China National Knowledge Infrastructure Database(CNKI),China Science and Technology Reader's Digest Database(VIP),Wanfang Data,Chinese Biomedical Literature Database(Sino Med)were retrieved from January 1978 to December 2018. The clinical observation and study literatures on the treatment of ischemic stroke with the combination of traditional Chinese medicine and Western medicine were retrieved in the four databases. After standardized and hierarchical collection and processing of all syndromes,treatment methods,prescriptions and other information in the literatures,a database of syndrome elements and treatment of ischemic stroke was established. Syndrome factors and treatment methods were analyzed by scale evaluation and hierarchical classification methods. Kendall's tau-b correlation analysis,principal component analysis and other statistical methods were used to describe the correlation and distribution of syndrome factors and treatment methods of ischemic stroke. Result:The results of heterogeneity analysis showed that the included literatures were homogeneous and could be combined with subsequent statistics. A total of 450 syndromes and treatment methods were included in this study,and 1 287 single syndrome elements and 1 562 single treatment methods were obtained after unified and standardized splitting. Besides the corresponding syndrome elements and treatment methods,phlegm-dampness-invigorating Qi(-0.52) and Qi deficiency-invigorating Qi(-0.56) were also highly correlated. The study team represented the importance of syndrome and treatment elements with class Ⅰ,Ⅱ,Ⅲ from high to low. Qi deficiency,blood stasis and fire heat,phlegm,viscera excess were class Ⅰ syndrome elements;Yin deficiency,endogenous wind were class Ⅱ syndrome elements;Yin deficiency and Yang deficiency were class Ⅲ syndrome elements;Removing phlegm dampness,clearing heat,clearing the hollow viscera and extinguishing wind,promoting blood circulation to remove blood stasis,tonifying Qi were class Ⅰ treatment of ischemic stroke,and removing phlegm dampness,clearing heat,clearing the hollow viscera were more likely to appear simultaneously; and extinguishing wind,activating blood circulation and removing blood stasis,and benefiting Qi were more likely to appear simultaneously. Nourishing Yin and regulating Qi were class Ⅱ therapies of ischemic stroke,which were highly correlated and often appear simultaneously. Inducing resuscitation,tonifying Yang and dredging collaterals were class Ⅲ,Ⅳ,Ⅴ therapies. Conclusion:Qi deficiency,blood stasis,phlegm dampness,fire heat and viscera excess were the main syndromes of ischemic stroke,while Qi deficiency and blood stasis,phlegm heat and viscera excess were the main syndromes. Eliminating phlegm and dampness,clearing heat,clearing the hollow viscera,promoting blood circulation and removing blood stasis,extinguishing wind and benefiting Qi were the main therapies for the treatment of ischemic stroke. In clinical treatment for ischemic stroke,the therapies for relieving phlegm and dampness,clearing heat and relieving organs are often used in combination,and the therapies for promoting blood circulation and removing blood stasis were often used in combination with the therapies for invigorating Qi and extinguishing wind for the synergistic effect.

ABSTRACT

Objective:To explore the mechanism of Bushen Huayu Shengxin decoction in delaying senescence of bone mesenchymal stem cells(BMSCs) by improving cellular microenvironment and regulating p16/pRb and p53/p21 signaling pathways. Method:The cells were cultured in serum-free 1640 medium and hypoxic cell workstation for 24 hours to establish the cell model of ischemic-hypoxic microenvironment in vitro, then randomized into control group (with complex medium), model group (with complete medium), and treatment group (with serum medium-containing Bushen Huayu Shengxin decoction), and all were cultured in hypoxic cell workstation for 24 hours. The normal group was added with control culture for complete medium, The cell cycle of BMSCs was detected by flow cytometry, the expressions of p16INK4a, p53, p21 and Survivn, cysteine aspartic acid protease-3 (Caspase-3), polyadenosine diphosphate ribose polymerase (PARP) mRNA were analyzed by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), and the levels of β-catenin protein and glycogen synthase kinase-3β(GSK-3β) protein were detected by Western blot. Result:Compared with the normal group, the proportion of S phase cells increased, while that at the G0/G1 phase decreased significantly in the model group (P<0.05). Compared with the model group, the proportion of S phase decreased, whereas that at the G0/G1 phase gradually increased in the treatment group (P<0.05). Compared with the normal group, mRNA expressions of p16INK4a, p53, p21 and Survivn, Caspase-3, PARP in the model group increased significantly (P<0.05). Compared with the model group, mRNA expressions of p16INK4a, p53, p21 and Survivn, Caspase-3, PARP in the treatment group decreased significantly (P<0.05). Compared with the normal group, protein expressions of β-catenin and GSK-3β in the model group increased significantly (P<0.05). Compared with the model group, protein expressions of β-catenin and GSK-3β in the treatment group decreased significantly (P<0.05). Conclusion:Bushen Huayu Shengxin decoction could delay the senescence of BMSCs by improving ischemic-hypoxic microenvironment and regulating p16/pRb and p53/p21 signaling pathways.

ABSTRACT

OBJECTIVE@#To investigate the value of preoperative three-dimensional image reconstruction in the treatment of ureteropelvic junction obstruction (UPJO).@*METHODS@#We reviewed data on 40 patients (22 male cases, and 18 female cases) diagnosed with UPJO in Peking University First Hospital from May 2017 to April 2019. The median age was 26.5 years (IQR 23.25-38.75) years. There were 11 patients complicated with ectopic vessels, 14 patients with kidney stones, 3 patients with horseshoe kidney, and 6 patients with obstruction after pyeloplasty. All the patients underwent preoperative enhanced CT scan, and the CT data were reconstructed into three-dimensional image models. The obstruction position of ureteropelvic junction and the relationship between ureteropelvic junction and blood vessels and organs were observed by three-dimensional models to assist planning surgery. Thirty-seven patients underwent laparoscopic pyeloplasty (including 3 cases combined with pyelolithotomy with flexible cystoscope, 1 case combined with pyelolithotomy by sun-style cystoscope, 1 case with laparoscopic ureter resection and anastomosis, 3 cases of laparoscopic pyeloplasty of horseshoe kidney), 2 patients underwent laparoscopic ventral onlay lingual mucosal graft ureteroplasty, and 1 patient underwent robot-assisted laparoscopic pyeloplasty.@*RESULTS@#Three-dimensional CT image clearly showed the relationship between the obstruction of ureteropelvic junction and blood vessels and organs after three-dimensional reconstruction. The type, diameter, position and direction of the ectopic vessels could be observed clearly before operation according to the three-dimensional reconstruction model, and the number, size, location and shape of renal calculi or other masses, the number of involved renal calyces and the anatomical distribution in the renal pelvis and calyces could be also evaluated preoperatively. After comprehensive analysis of the above information, individualized operation plans were performed on the patients, all the 40 cases were successfully completed with the surgery without any transfer to open surgery. The average operative time was (129.91±37.90) min (range: 75 to 273), the average blood loss was (48.1±78.0) mL (range: 10 to 400), the average hospitality was (5.04±1.99) d (range: 2 to 10), and the average postoperative drainage time was (3.8±1.4) d (range: 2 to 8).@*CONCLUSION@#The preoperative three-dimensional image reconstruction has a high clinical value in the treatment of ureteropelvic junction obstruction, and it is of great help to assist surgery planning and is worthy of further clinical promotion and application.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the mechanism that the formulas for activating blood and resolving stasis can regulate hemopoietic stem cell to produce new blood.</p><p><b>METHOD</b>Rats were established animal model of acute cerebral infarction by referencing Olivette' method. They were randomly divided into model group, the group of the high, middle, low dose of the formulas for activating blood and resolving stasis. Each group and then wasrandomly divided into subgroups by 1, 3, 7, 14, 28 d. Xuesaitong capsule was formulated into 20, 40, 60 g x L(-1) with normal saline. The rats were given gavage drugs once a day until the experient ended, and the model group was administrated by intragastrical perfusion of normal saline. ELISA was used to detect the expression of SCF in peripheral blood and bone marrow among different groups at different time points. Flow cytometry was used to observe the changes of CD117 in blood and bone marrow.</p><p><b>RESULT</b>The CD117+ HSC and SCF concentration in peripheral blood and bone marrow of model group were increasing during 1-14 d,there was a peak on the 14th day, then the expression was reducing. CD117+ HSC and SCF concentration rising trend in the group of the high, middle dose of the formulas for activating blood and resolving stasis was preceded model group (P < 0.05).</p><p><b>CONCLUSION</b>Activating blood and resolving stasis can regulate hemopoietic stem cell to produce new blood, and it is through the regulation of CD117+ HSC number to achieve the purpose.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of postoperative enteral immunonutrition on inflammatory response and immunologic function in patients with gastrointestinal tumor.</p><p><b>METHODS</b>Clinical data of 106 gastrointestinal malignant tumor patients with malnutrition who were treated in the Department of General Surgery, the People's Hospital of Cangzhou in Hebei province from January 2008 to June 2010 were prospectively collected. Patients were randomized into two groups, including enteral immunonutrition group(n=53) and common enteral nutrition group(n=53). Related immunological indices and C-reaction protein were measured on preoperative day 5 and postoperative day 1 and 9.</p><p><b>RESULTS</b>The general information and preoperative immunological indices were comparable between the two groups(P>0.05). On postoperative day 9, levels of CD4, CD4/CD8, IgG, lymphocyte, NK cells, and complement C3, C4, and CH50 in the enteral immunonutrition group were higher than those in common enteral nutrition group. Serum C-reaction protein level was lower than that in control group, and the difference was statistically significant (P<0.05). Postoperative infection rate was 3.8%(2/53) in enteral immunonutrition group, significantly lower than that in control group with an infection rate of 15.1%(8/53)(P<0.05). The mean postoperative hospital stay of the two groups were (8.1±1.1) d and (9.2±2.1) d, respectively, and the difference was statistically significant(P<0.05).</p><p><b>CONCLUSION</b>For gastrointestinal malignant tumor patients with malnutrition, the use of enteral immunonutrition can alleviate the postoperative trauma and inflammatory response, improve the immune function, thus can reduce the occurrence of postoperative infection, and accelerate patient recovery.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To profile the protein expression in activated rat hepatic stellate cells (HSCs).</p><p><b>METHODS</b>Primary rat HSCs were isolated and cultured in vitro. After 10 days in vitro culture, the HSCs were activated. Total protein extracted from these activated HSCs were digested, and the obtained peptides were analyzed by using online 2D nanoLC-MS/MS. The identified proteins were classified according to their distributions and functions.</p><p><b>RESULTS</b>1014 proteins were identified from 50 microg HSCs protein extract, the molecular weights of these proteins ranged from 7832 Da to 588,364 Da. Most of these proteins resided in nucleus, cytoskeleton, mitochondrion and endoplasmic reticulum. And these proteins were mainly involved in nucleic acid metabolism, organelle organization, signal transduction and energy generation. Among these proteins, alpha-smooth muscle actin, vimentin and desmin were specifically expressed in activated HSCs.</p><p><b>CONCLUSION</b>To the best of our knowledge, this is the most comprehensive protein expression profile of activated rat HSCs.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the anti-depressive mechanism of Shugan Jiannao Tiaoyu tablets (SJTT) and provide evidence for clinical application.</p><p><b>METHOD</b>Evaluate the behavioral index changes of rat model through the methods of motionless-time of swimming by force, and the consumption of sucrose, etc, meanwhile, use transmission electron microscope (TEM) to observe the cell ultrastructure and study the influence of the hypothalamic corticotrophin releasing hormone (HCRH) gene expression with the RT-PCR technique.</p><p><b>RESULT</b>SJTI could reduce the hippocampal neuronal damage caused by stress, protect the brain injury of the depressive rats. SJTT could down-regulate model rats' HCRH. Compared with the model group (P < 0.05), which indicates that SJTT could inhibit the hyperfunction of HPA Axis by down-regulating model rats' HCRH, to achieve the antidepressant effect.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study on the regulatory mechanism of lipid metabolism disorders in the blood fat of hyperlipemia rat model with Olive Antihyperlipidemia capsule, and do systematic observation on the functions of this medicine on low And high density lipoprotein receptor in rat liver gene expression, and then to clarify the mechanism of action of this medicine on treating hyperlipemia.</p><p><b>METHOD</b>To select SD rat as investigated subject. The hyperlipemia rat models were made with feeding high-fat forage and were randomly divided into six groups based on the total cholesterol level at the ratsfasting for 12 hours: group A, B, C, D, E and group F. The samples in the research were collected and analyzed the changes of LDLR/SR-B1 gene expression in rat's liver by RT-PCR.</p><p><b>RESULT</b>Olive Antihyperlipidemia capsule can markedly enhance LDLR/SR-B1 gene expression in rat's liver and finally accomplish the purpose of reducing blood fat. The experiment shows this medicine has the remarkable effect on hyperlipidemia and proved the theoretical system of treating hyperlipemia for curing the liver is correct.</p><p><b>CONCLUSION</b>Olive Antihyperlipidemia capsule has an applicable value on preventing the cause, enhance LDLR/SR-B1 gene expression in rat's liver and finally accomplish the purpose of reducing blood fat and development of hyperlipemia and its complications.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the survey methods for the cultivated medicinal plant resources such as Panax notogingseng based on remote sensing.</p><p><b>METHOD</b>Through the preliminary study by remote sensing technique and field survey in the study area, the optimal methods in the procedure were chosen for the survey.</p><p><b>RESULT</b>In this study,a set of survey method for P. notogingseng based on remote sensing technique was founded, which included the remotely sensed data-source selecting, the remote sensing image processing, interpretation and validation etc.</p><p><b>CONCLUSION</b>With different resolution remotely-sensing data at the study area, the area, the natural storage and the yield of P. notoginseng were calculated. For Maguan county, with SPOT5 (5 m), the relative right rate of judgement, the relative precision of area and the relative precision of yield with statistics was 92.7%, 94.4% and 78.2% separately, and with TM (30 m), the relative right rate of judgement, the relative precision of area was only 71.4% and 58.8%, though, for Qiubei county, with TM (30 m), the relative precision of area with statistics could reach 74.7%, which showed that choosing appropriate remotely-sensing data and processing methods for different area, the remote sensing technique is feasible for resource survey of cultivated medicinal plants such as P. notoginseng.</p>

ABSTRACT

<p><b>OBJECTIVE</b>The expression of C/EBPalpha protein and mRNA during automatically activation process in primary cultures of HSCs were observed in order to explore its possible association with the proliferation and activation of HSCs.</p><p><b>METHODS</b>Immunocytochemistry, Western blot and RT-PCR were used to evaluated the expression of C/EBPalpha protein and mRNA; as well as the expression of alpha-SMA, Desmin, MMP2, type I procollagen (alpha1). The eukaryotic vector harboring the full length cDNA of C/EBPalpha was transfected into activated HSC, then immunocytochemistry was applied to confirm the transfection and evaluate the effect of transfection on the proliferation of HSC by calculating the PCNA-positive cells. The morphological changes of HSC were observed by use of phase-contrast microscope.</p><p><b>RESULTS</b>Constitutive expression of mRNA and protein of C/EBPalpha were detected in primarily cultured HSCs, and the protein was seen in both nuclei and cytoplasm with the latter being dominant. Their expression levels reached highest at day 2 of the culture, then decreased gradually when continually cultured to the day 4, 7, 10, on the other hand, the expression of alpha-SMA, MMP2 and ColI(alpha1) increased steadily. Transient transfection was verified by the fact that much more and stronger C/EBPalpha stain was observed in transfected HSCs than in void-vector transfected cells. In C/EBPalpha gene transfected HSCs, the number of PCNA-positive cells dramatically decreased compared with the void-vector transfected cells 24h after transfection. In addition, the C/EBPalpha gene transfected HSCs died 36 h after transfection, a few surviving cells became longer and thinner in morphology, however the void-vector transfected cells almost all remained alive.</p><p><b>CONCLUSIONS</b>C/EBPalpha was likely involved in the HSCs activation, and over-expressed C/EBPalpha by transfection had inhibitory influence on the proliferation of cultured rat HSCs.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of RAR-beta transfection plus treatment with the corresponding ligand ATRA on the proliferation and phenotype of platelet-derived growth factor (PDGF)-activated hepatic stellate cells (HSC).</p><p><b>METHODS</b>PDGF-activated hepatic stellate cells of rats were transfected with eukaryotic expression vector pCMV-script-RAR-beta, which was verified by western blot. The proliferation of transfected HSC was assayed by BrdU incorporation as well as MTT methods. Their phenotype (alpha-SMA and desmin) was observed by immunocytochemistry assay with image analysis and RAR-beta protein expression was detected by western blot.</p><p><b>RESULTS</b>Transfection of RAR-beta gene and treatment with ligand ATRA could increase the expression of RAR-beta protein for at least 144h and inhibit the proliferation and the expression of alpha-SMA and desmin in PDGF-activated HSC. Significant statistical differences were perceived comparing with sham-transfected, only-PDGF treated, non-ligand treated and irrelevant ligand-treated HSC.</p><p><b>CONCLUSIONS</b>Transfected with RAR-beta gene as well as using related ligand ATRA could suppress the proliferation and reverse the activation phenotype of activated HSC.</p>
